Insurance Coverage
I believe that lactation and infant feeding support should be a basic part of healthcare provided to you by your insurance. I am always working to get in network with more carriers, so if you don't see yours below, please let me know so that I can get on it!

For families with insurances not listed below, I will provide a superbill for you to submit to your insurance for potential reimbursement of my out-of-pocket rates. Payment will be due at the time of service. Your insurance carrier will reimburse you directly if superbill submission is approved.
My out-of-pocket rates are $250 for initial visits, and $150 for follow up visits. For bottle feeding only consults (no pump support) my fee is $150.

If you are insured by Aetna, UHC PPO + HMO, Blue Cross of Illinois or Rhode Island, First Health, First Choice Health, Imagine Health, VA CCN, Meritain,Multiplan, PNOA, Sutter Health, UHC, or Velocity PPO please follow this link to get pre-approved for your visits. If approved, all visits will be fully covered at no cost to you.

​If you are insured by Partnership Healthplan of California, Alliance for Health, San Francisco Health Plan, Health Net, Community Health Plan, Gold Coast Health Plan, Contra Costa Health, or Medi-cal plans with Health plan of San Joaquin, Mountain Valley Health Plan, DHCS/Medi-cal Direct, Health Plan of San Mateo, CalViva, Anthem, Sharp Health Plan, L.A. Care, or Blue Shield of Califonia (Promise plan only) please follow this link to confirm your coverage. Indicate myself, Angelica Merzigian, as your "doula preference".

Families covered by Blue Cross Blue Shield of California plans, if provided by your employer, or if you purchased a commercial plan, your lactation care may not be covered. Beginning Summer of 2025, many plans stopped paying out for lactation codes, but not all. You can reach out to your member services line to ask if any lactation codes are covered, if you need pre-authorizations, which codes specifically, and is there a limit. If no out of hospital benefits are available to you, ask about doula coverage, codes and limits. Doula care typically includes lactation support. Exceptions are plans obtained via Medi-cal or Promise plan.
